This must be completed by a parent or legal guardian before the child can attend afterschool for the 2026-2027 school year.  One application per student please. Do not put siblings together on one form. 

We're reaching out to ask for a few minutes of your time to complete an important parent survey. As part of our Title I School-Wide Program and Consolidated Federal Grants, this survey helps us understand the needs of your child’s school and guides our planning for the upcoming school year.

Your input will also help shape Ashe County Schools’ Strategic Plan for 2026 and beyond. The survey focuses on academics, parent and family engagement, and school culture—and your voice truly matters in making our schools the best they can be.

As we prepare for the 2026-2027 school year, we are beginning to collect information from families to support efficient planning and routing.

Even if a paper transportation form has already been completed for this upcoming year, we are asking that all families please complete this online form as well. This will help us ensure consistency and accuracy across all student transportation records.
